To successfully complete this Form, you will need a digital copy of the following documents:

1. Proof of Baptism: At least one (1) parent needs to be a baptized Catholic to baptize their child into the faith. We require a copy of their Certificate of Baptism (one or both parents);
2. Child’s Birth Certificate;
3. Confirmation Certificate of godparents.

To obtain a copy of the updated baptism certificate, the god parents just have to call the church they were baptized at and they can get you an updated copy! The Updated Baptismal Certificate shows all the sacraments received (except for First Holy Communion).
